Review your blood sugar log, and identify where there appears to be a pattern where blood sugars are not meeting target goals.

Weigh yourself and compare with target weight.

Jot down question areas of concern – need for prescription refills, need for referrals, etc. Ask if it’s time for 3-month (A1C) test.

At Every Office Visit

Start discussions with your most serious concerns.

Review blood sugar log with doctor.

Take off shoes and socks to get feet checked.

Ask for your weight and blood pressure numbers and write them down in log.

If given any lab results, ask for specific numbers and/or a copy of results.

Refer to notes for areas to review with the doctor.

If medication is ordered, have doctor explain it

Fill in your Diabetes Checkpoints sheet and identify all areas that need improvement.

Identify ways to improve.

At Least Every 3 - 6 Months

Schedule a follow-up appointment with your doctor.

Ask the doctor to order the 3-month (A1C) test. Other:

Have cholesterol, LDL, HDL, Triglycerides checked.

Have a dilated eye exam.

Have your kidney function checked.

Get a flu shot.

Identify health professionals who are on your team who can help. Include ECDA on your list of supporters.

Communicate any new development or areas that need follow-up.

Check with physician as to frequency of pneumonia shot (usually just received once).
